Most of the over 30 hospice agencies in Connecticut are members of the Connecticut Association for Home Care & Hospice (CAHCH) which supports their efforts to provide

high quality and accessible services through collaboration, leadership, provision of information, advocacy and education.

National Hospice and Palliative Care Organization  www.nhpco.org

The National Hospice and Palliative Care Organization (NHPCO) is the oldest and largest nonprofit membership organization representing hospice and palliative care programs and professionals in the United States. NHPCO represents over 80% of hospices nationwide.  Over 90% of the patients served in 2002 received hospice care by NHPCO members.

NHPCO offers memberships to hospices, palliative care programs, grief and bereavement centers, education and research organizations, foundations, home health agencies, companion services, medical supply companies, pharmaceutical organizations, staffing agencies, software vendors, and health care consultants.

 

Hospice and Palliative Nurses Association

HPNA is a membership organization with individual membership levels for the Advanced Practitioner, Generalist Nurse, LP/VN, Nursing Assistant and Associate (non-nursing). HPNA offers members specialty nursing resources, information, and direction.  Evidence-based educational tools are available to help with complex symptom management, managing grief & bereavement, peer support, having difficult conversations, educating about hospice or palliative care.

 

American Academy of Hospice & Palliative Care Medicine

The Academy is the professional organization for physicians specializing in hospice and palliative medicine. Membership is also open to nurses and other healthcare providers who are committed to improving the quality of life of patients and families facing life-threatening or serious conditions.

City of Hope Pain/Palliative Care Resource Center (COHPPRC)

The purpose of the COHPPRC is to serve as a clearinghouse to disseminate information and resources to assist others in improving the quality of pain management and end of life care. The COHPPRC, established in 1995, is a central source for collecting a variety of materials including pain assessment tools, patient education materials, quality assurance materials, end of life resources, research instruments and other resources.
